Online scheduling and forms

*Please read below before requesting your appointment.

 

You will be making an appointment request.  We will notify you that your selected time/day is available.

 

• Important:  Online requests for same-day appointments will not be accommodated.  Please call the office for same-day availability.

​

• If you are new to our office, we recommend filling out your patient forms before you come to your appointment (see below).  Also, arrive 15 minutes before your scheduled time.

 

• If you need to schedule a dilation or a follow up visit only, please call the office.

​

• If you are new to wearing contact lenses, please call our office to schedule, as a special training class will need to be arranged.
